The MDPCP Program Management Office (PMO) in collaboration with Medicalincs LLC provided this in-person, MDPCP Advanced Primary Care Staff Training. This comprehensive, hands on, interactive, one day training program was designed to enhance practice staff’s expertise with the functions of Advanced Primary Care, and how to implement change to meet the MDPCP care delivery requirements, and ultimately patient health and experience outcomes.

SESSION 1: Maryland Primary Care Program (MDPCP) - OVERVIEW

Session objectives: 


 Understand the "why" of MDPCP & how the work that you do in MDPCP will impact the delivery of health care services 


 Describe the MDPCP framework 

SESSION 2: Change Management & PROCESS IMPROVEMENT

Session Objectives:


 Understand the process of change & where they are in the process

 Describe ways to adopt to changes while coordinating patient care

 Understand QI improvement process – PDSA; workflows, people, process, technology/tools

 Understand how each individual plays a role in the quality improvement process

session 3: team-based care

 Session Objectives:


 Understand the principles & benefits of team-based care 

 Understand care team structures 

 Understand how to navigate team-based care 

    o Building positive relationships on a team 

    o Communication with team members 

    o  Dealing with team conflict

SESSION 4: Phase I - Patient Care BEFORE Office Visit

Session Objectives


Understand how the following care coordination concepts in this phase of patient care:

  High-level workflow reviews key functions (People, Process,   Technology/Tools)

  Working on interdisciplinary teams & team member   roles & benefits of team care

  Empanelment (including EMR tools)

  Participating in team huddles 

   Know what minimum data sets are necessary for a warm hand-off

  Identify proper and secure documentation of minimum data sets necessary for each transition of care

  CRISP Tools for Risk stratification 

session 5: PHASE ii - patient care during the office visit

Session Objectives:


Understand how the following care coordination concepts in this phase of patient care:   

  High-level workflow reviews key functions (People, Process,   Technology/Tools – EMR; CRISP, Office etc.

   Working on interdisciplinary teams & benefit of team-based care 

   Annual   Wellness Visits (AWV) and EMR Tools

   EMR   Tools for Screening (BH, CM etc.)

   Other Screening tools – Risk assessment, SBRIT,   PHQ 2/9,l etc. 

   Know what minimum data sets are necessary for a warm hand-off

   Identify proper and secure documentation of minimum data sets necessary for each transition of care 

session 6: Phase III - Patient Care AFTER Office Visit

Session Objectives:


Understand how the following care coordination concepts in this phase of patient care: 

  High-level workflow reviews key functions (People, Process,   Technology/Tools – EMR; CRISP, Office etc.

   Working on interdisciplinary teams & benefits of team care

    Know what minimum data sets are necessary for a warm hand-off

    Longitudinal Care Management 

    Care Plan Basics

    Episodic Care Management, TCM, patient discharge review & follow up 

SESSION 7: Using CRISP Services & Leveraging Claims Reports

  Session Objectives: 


   Understand how to create and post CRISP Care Alerts

   Understand action(s) to take or escalation process for care alerts (these are different from CRISP Care   Alerts)

   Understand how to use the CRISP report to identify high cost-high volume providers

   Overview on updates in Q3 and Q4 

session 8: Self-Management & On-line self-help programs

 Session Objectives: 


 Understand what patient self- management is

 Know available and relevant tools to assist patients with self-management & maintaining a resource registry

 Know tools for patient engagement and assessment such as PAM 

SESSION 9: Social Determinants/Needs – Services & RESOURCES

Session Objectives:  


   Define social  determinants of health.

   Understand  determinants that may affect a patient’s health (such as gender, race, class,  ethnicity, and place of residence)

   Describe the tools that healthcare workers can use to help patients and their families access needed resources

   Be able to help patients talk to their doctors and prepare them for medical visits

SESSION 10: PATIENT & FAMILY ADVISORY COUNCIL (PFAC)

 Understand what PFAC is 


  Understand how PFAC captures patient/caregiver feedback to improve care delivery 

Session 11: MOtivational Interviewing

 Session Objectives:  


   Understand motivational Interviewing techniques, to better

   Understand the following concepts: 

      o  Recognizing patients’ families’ cultural needs/factors that may    affect their choices or engagement 

      o  Communicating with patients and families in a culturally competent manner
