2020 MDPCP Advanced Primary Care Academy

The MDPCP Program Management Office (PMO) in collaboration with Medicalincs LLC offers Advanced Primary Care Staff Training Sessions. This comprehensive, interactive, 3-hour virtual program was designed to enhance practice staff’s expertise with the functions of Advanced Primary Care; especially during COVID-19.

Part II: COVID: Road Map to Recovery (Testing)

Session objectives:

  • To support MDPCP primary care practices in putting together a COVID-19 testing strategy for patients, either within the practice (if appropriate) or externally​
  • Understand Reimbursement Codes by Payer for COVID-19 Testing

TRAINING | 2020

Session 1: Intro & Overview of MDPCP

Session objectives:

  • Understand the “why” of MDPCP & how the work that you do in MDPCP will impact the delivery of health care services
  • Describe the MDPCP framework
  • Describe the impact of COVID-19

Session 2: Change Management| Team Based Care| Quality Improvement

Session objectives:

  • Understand the process of change & where they are in the process
  • Understand Team Based Care in MDPCP
  • Understand QI improvement process – PDSA; workflows, people, process, technology/tools

Session 3: The Patient Journey – Before, During & After the Visit

Session Objectives:

Understand the following care coordination concepts:

  • Empanelment & Risk Stratification
  • Team huddles & Warm Handoffs
  • Minimum data sets are necessary for a warm hand-off
  • Annual Wellness Visits (AWV)
  • EMR Tools for Screening (BH, CM etc.)
  • Longitudinal Care Management & Care Plans

Session 4: Phase III | Social Determinants & Patient Engagement (PFAC) | Q & A

Session Objectives:

  • Episodic Care Management, & patient discharge review & follow up
  • CRISP Tools
  • Care Collaborative Agreements
  • Define social determinants of health.
  • Understand determinants that may affect a patient’s health
  • Social needs resources
