Healthlincs Care Transformation Organization (CTO) approved by and in partnership with the Maryland Department of Health and Centers of Medicare and Medicaid (CMS) to implement the Maryland Primary Care Program (MDPCP). MDPCP is a value-based care initiative designed to provide patient-centered care to Medicare fee for service (FFS) patients who live in Maryland.
Our CTO provides services that are integral to meeting the care transformation requirements, and our partner providers benefit from the CTO clinical & technical assistance tailored to their needs. We help practices with personnel and expertise to:
– Transform the delivery of primary care
– Meet utilization and quality metrics
– Align with the hospitals and improve care coordination
– Ensure appropriate connections with other providers
– Assess and assist practices to manage care
– Access non-traditional workforce to enhance care management
– Use data to control total cost of care

The goal of the program is to provide integrated, community-based care coordination services for individuals living with these rare & expensive conditions.
Medicalincs (as a REM Provider) provides individuals in the REM program with specialized care coordination services that will improve their access to appropriate, high quality, cost-effective services, and also improve the overall individual care experience.
