WHAT IS THE REM PROGRAM?

The Rare and Expensive Case Management (REM) Program is a case managed service alternative, that is part of the Maryland Medicaid HealthChoice Program. The REM Program support individuals with specified rare & expensive conditions. Medicalincs (as a subcontractor to The Coordinating Center) is a REM Provider.

SERVICES COVERED UNDER REM PROGRAM

The goal of the program is to provide integrated, community-based care coordination services for individuals living with these rare & expensive conditions.

Medicalincs provides individuals in the REM program with specialized care coordination services that will improve their access to appropriate, high quality, cost-effective services, and also improve the overall individual care experience.

– Must be eligible for Medical Assistance (Medicaid)

– Must have a REM qualifying diagnosis

– REM Intake Referral Form

Referral

An individual can be referred by anyone for the REM program. Medicaid managed care organizations (MCOs) refer individuals when they identify those with REM qualifying diagnoses whose needs are outside the scope of the managed care organizations’ benefits and services.

HOW TO APPLY

Review eligibility criteria
Meet with your physician. The REM application must be completed and signed by the referring physician and include proper medical documentation supportive of the qualifying diagnosis

Submit completed form and all supporting clinical information to the:
